We are seeking a Band 4 Nursing Associate to support the Lead Nurse in delivering an expert Hospice at Home service, enhancing the experience of babies, children, young people and families requiring palliative and end of life care. Key responsibilities include Working in children and young peoples homes as part of the Hospice at Home service. Supporting the Registered Nurse in delivering safe, person centred care, often working independently with an agreed plan of care. Delivering excellent short break, palliative and end of life care, including care after death. Participating in assessment, planning, implementation and evaluation of care under the direction of the Registered Nurse. Engaging with families to understand needs, preferences and choices, contributing to risk assessments and care planning. Providing a high standard of holistic care. Adhering to the NMC Code and working within your scope of practice. Taking responsibility for medication administration, storage and return in line with policy. Maintaining accurate, contemporaneous records and handling information in line with legislation and policy. Communicating sensitive information effectively and using appropriate strategies to support person centred care, equality and diversity. Using SystmOne appropriately. Acting independently and collaboratively to uphold the rights of individuals. Liaising with the wider multidisciplinary team and external agencies to ensure continuity of care.
