Crynodeb
To quality assess and monitor completed review health assessments for Children in Care including those with complex needs or identified as hard to reach. To work in collaboration with service providers across the local health economy and with partner agencies within and outside the county, to ensure that the health of Children in Care is a priority of practice and service provision. To undertake high quality, complex, timely health assessments for Children in Care, young people and care leavers including asylum seekers and work with interpreters as required To engage with hard to reach children and young people in order to effectively ensure their health needs are identified and met. To provide expert direct clinical practice plus advise and support practitioners across the health economy in health matters relating to Children in Care, which is consistent with current professional standards and promotes best practice, as single agency and multi-agency events, supported by the Integrated Care Pathway. The post holder will provide expert advice, training and supervision to those who work directly with looked after children e.g. foster carers and social workers.
