• To work as an autonomous practitioner within an integrated team, supporting the delivery of the EPUT vision of patients being seen by the right person, with the right skills at the right time.
To lead a team of mixed skill community nursing staff to manage appropriate care for patients in their own home/community setting
• To manage the complex role of Community Nurse over a defined area, with a commitment to the health improvement of individuals, families and the community.
The post holder is required to comply with all EPUT Policies and Procedures and the NMC Code.

An exciting opportunity has arisen for a highly motivated and innovative Registered General Nurse to lead an integrated community nursing team under the guidance of the Modern Matron and Senior Community Lead Nurse in Castlepoint.
To lead a dedicated nursing service to include assessment, planning, treatment and delivery of nursing care in the community. This includes assessment of health needs, leading a team of staff and meeting the ever developing role of the community nurse. You will provide patient support in order to prevent unnecessary hospital admission, enabling them to stay at home where possible.
You will be responsible for the supervision, support and teaching of junior staff. You will be expected to work within the policies of the Trust and the NMC Code.
You will use a combination of skills and knowledge to identify health and nursing needs, develop strategies to promote health and independence whilst recognizing cultural beliefs and values of the individual. You will be expected to liaise closely with multi-disciplinary teams as part of collaborative working to support the case management of patients.
The successful applicant will be a 1st level nurse with 3 years community nursing experience who can demonstrate leadership and the ability to work within a changing environment, who is flexible and keen to develop as the service needs.

Responsible and accountable for the ongoing assessment, development, implementation and monitoring of health care thus ensuring that the highest standard of service and care is delivered in a consistent and safe manner, within the home and clinical setting.
Undertake a range of clinical duties/nursing procedures and participate in the development of guidelines and protocols and ensuring all policies are implemented and adhered.
