PRIMARY DUTIES & AREAS OF RESPONSIBILITY 1. Works autonomously in accordance with NMC guidelines, to manage his/her caseload of patients. 2. Develop and undertake nurse led clinics and prescribing (if qualified) with support of consultants and l lead nurse. 3. Act as key worker for cancer patients and undertake holistic needs assessment in line with national and local guidance 4. Communicates highly sensitive and emotive condition related information to patients, relatives e.g. terminal illness, bereavement etc. Participates in complex ethical decision- making process 5. To act as the patients advocate on a frequent basis and provide expert advice on treatment options to enable patients to make informed choices 6. Provides continuity of care throughout the patients journey and liaises across organisations 7. Develops specialised programmes of care and provides highly specialised advice and written and oral information to patients, relatives 8. To act as a key communication resource for the multidisciplinary team 9. Provides clinical advice and support to healthcare professionals within and across organisational boundaries 10. To act as a role model for excellence 11. Provides support for distressed patients/carers and bereaved relatives. Identifies patients at risk and refers to appropriate agencies 12. Reviews patients independently and initiates changes in the management of care 13. Maintain accurate patient records and ensure all relevant information is documented in the patients notes 14. Promote and participate in support groups and other patient and public involvement initiatives to inform service development 15. Work collaboratively with other healthcare professionals and agencies involved in the discharge process to ensure patients are transferred into primary care in a timely and appropriate manner 16. Admit patients using own clinical judgement and convey relevant information to other members of the specialist team 17. Act as a change agent using own specialist knowledge of products and resources 18.
