Clinical Pharmacist (General Practice) Location: DeMontfort Medical Centre, Evesham Hours: Part-time, up to 24 hours per week Salary: £50,000 £60,000 (pro rata, dependent on experience) Closing Date: Friday 27th March 2026 About Us DeMontfort Medical Centre is a modern, purpose-built GP practice in Evesham with excellent facilities, including onsite staff parking. We are seeking an experienced and enthusiastic Clinical Pharmacist (Independent Prescriber) to join our multidisciplinary team. This is an exciting opportunity to take on a key clinical role and contribute to safe, effective, and efficient medicines management within the practice. About the Role The Clinical Pharmacist will support our GPs, nurses, and wider healthcare team to optimise medicines use and improve prescribing safety. You will work both independently and collaboratively, acting as a key source of specialist medicines knowledge within the practice. Key Responsibilities Clinical Care & Medicines Optimisation Conduct structured medication reviews (SMRs) for patients with long term conditions. Provide expert clinical advice on medicines and prescribing. Support safe prescribing, including high-risk drug monitoring. Undertake medication reconciliation following hospital discharge or specialist input. Identify and manage medication-related risks and contribute to patient safety initiatives. Provide patient counselling on medicines use, side effects, adherence, and treatment plans. Prescribing & Medicines Management Manage repeat prescriptions and medication queries. Ensure prescribing follows NICE guidance, local formularies, and NHS prescribing policies. Assist with cost-effective prescribing initiatives and medicines management projects. Support high-risk medication monitoring and local medicines management schemes. Team & Practice Support Provide education, training, and guidance to clinicians and staff. Work collaboratively with community pharmacies, hospitals, and the Primary Care Network. Contribute to clinical audits, QOF targets, service development, and quality improvement work. Support development of practice policies, protocols, and clinical governance systems.
