Overview of general responsibilities. Within the Hospice to: To work as a member of the Wellbeing Team and Multidisciplinary Team under the supervision of the Wellbeing Lead. Maintain and respect the confidentiality of the patient and their family. At all times act in a professional, pleasant, polite manner, fostering an atmosphere of co-operation and respect with a can do approach Communicate and listen to patients and their families reporting their needs and concerns as appropriate to qualified staff. To assist in the delivery of toileting. Provide 1:1 sessions which include what matters to you discussions, goal focused practice of tasks, relaxation, reminiscence, social opportunities. Deliver Group sessions (as per training and delegation). Support Group facilitators where required. Escorting participants to and from sessions and to their transport. Provide mobility and transfer assistance. Take phone calls, respond to enquiries and complete Health Questionnaires. Update SystmOne, Outlook Calendars and scheduling of participants. Identifying wellbeing needs of individual patients, through communications and through clinical handovers. Ensure infection control procedures are adhered to for safety and participant experience. Demonstrate work activities & duties to new staff and volunteers. Maintain general tidiness and housekeeping duties as required. Assisting with the serving and clearing of refreshments. Respect confidentiality at all times. Work within and adhere to hospice policies. Provide the highest quality of emotional, physical, spiritual, psychological, and social care to ensure the patients needs are met, in a safe manner. Be proactive and flexible in your approach and contribute positively to the ongoing development of Hospice Services. Collaborate in the assessments of patients, when required, to help shape their care delivery. Recognise changes in conditions of patients and swiftly and appropriately report back to senior members of the team. Collaboratively work with individuals support network to enable them to support the patient. Plan with registered professionals activities, interventions or treatments to achieve specified goals and implement care plans, where appropriate.
