We are seeking motivated and compassionate Pathway Navigatorsto join our specialistInterstitial Lung Disease (ILD) team. This is an exciting opportunity to play a pivotal role in supporting patients with complex respiratory conditions, ensuring they receive timely, coordinated, and person-centred care throughout their treatment journey.

As a Pathway Navigator, you will act as the first point of contact for patients and their families, guiding them through diagnostic, treatment, and follow-up pathways. You will work closely with clinicians, nurses, allied health professionals, and administrative staff to streamline processes and improve patient experience.

There are 2 posts available with 0.6 WTE based at North Manchester General Hospital and 0.6 WTE based at Manchester Royal Infirmary Hospital
• Deliver a high-quality administrative and secretarial service to the ILD team.
• Coordinate weekly MDT meetings, including listing newly diagnosed patients and recording treatment decisions.
• Book and manage outpatient appointments, ensuring smooth patient flow through the pathway.
• Track patients against national targets and NICE recommendations, escalating delays to prevent RTT breaches.
• Collect, record, and report patient data for audit and research purposes, including real-time reporting to external organisations.
• Coordinate diagnostic activity (radiology, bloods, physiology), reducing duplication and ensuring right patient, right test, right time.
• Manage the ILD waiting list in line with the Trust’s cross-site pathway.
• Act as the first point of contact for complex enquiries, handling sensitive information with professionalism.
