To lead and coordinate the development and delivery of End-of-Life Care using effective evidence-based practice in line with agreed national standards across South Tyneside and Sunderland NHS Foundation Trust, predominantly Sunderland Community setting. Lead and coordinate the development and delivery of End-of-Life Care using effective evidence-based practice in line with agreed national standards for patient care, safety and clinical practice Develop, deliver and influence End of Life care education and practice To use a range of teaching, learning, mentoring and coaching methods to enable staff development Facilitate the development of knowledge and practice across all staff. Provide clinical leadership, expertise and support to clinical staff regarding the management and care of patients at the end of life. Collaborate with other healthcare professionals/relevant agencies, to promote a seamless service and if appropriate a coordinated discharge to ensure continuity of care for patients at the end of life. Plan and lead audits to monitor progress of initiatives relating to end of life care and provide regular reports on request (including the Annual National Care of the Dying Audit for Hospitals). Project manage and lead the implementation of initiatives in end-of-life care. Support local implementation plans to deliver strategic initiatives such as Deciding right. Work collaboratively with key stakeholders to inform Trust, regional and national end of life care policy and practice. Work collaboratively with multi professional teams to ensure practice is efficient, effective and evidence based Support the development, delivery and monitoring of quality improvement targets, relating to End-of-Life Care Work collaboratively with the Specialist Palliative Care Team to deliver the end-of-life care agenda 14.
