BWA Health & Care Services Ltd (BlayCare) has been proudly supporting the Derby community for nearly 20 years. Due to continued growth, we are recruiting experienced and motivated Team Leaders/Senior Carers to join our dedicated and supportive team.

Community Care Team Leader / Senior Carer

We are looking for a compassionate and organised Community Care Team Leader to support the delivery of high-quality, person centred care to clients in their own homes. This role involves working closely with the care team to ensure excellent standards of care, compliance with policies and procedures and positive outcomes for service users.

Key Responsibilities

Deliver personal care and support to service users in their own homes
Work with the care team to ensure care is delivered to a high standard and in line with policies and procedures
Monitor and report care delivery and ensure completion of care and administrative tasks
Assist with staff rotas and communicate schedule changes to the care team
Identify and support training and development needs of assigned care team
Maintain accurate care records and documentation
Participate in team meetings, quality reviews and home visits
Conduct, support and contribute to service user care plans, reviews and manage ongoing care support
Build strong relationships with clients, families and external professionals
Report and escalate any concerns appropriately
Provide support to the care team and participate in the on-call rota
Respond to emergencies and undertake additional duties as outlined in care plans
About You

Passionate about delivering high-quality care
Strong communication and organisational skills
Ability to work well independently, within a team and support colleagues
Committed to maintaining accurate records and following policies and procedures
Achieved or working towards Level 3 NVQ in Health & Social Care (or equivalent)
Understanding of health and social care legislation and confidentiality
Experience in the community care
Knowledge of medication management and MAR sheets
Excellent communication and relationship-building skills
Confidence using care management systems (Access experience desirable)
