Crynodeb
To be accountable for the delivery of clinical services ensuring a high standard of care based on best practice to maintain patient/service user safety and provide an excellent experience. To coordinate the assessment of patients/service users with complex needs including individuals with unconfirmed diagnosis. To coordinate the development, implementation and evaluation of specialised programmes of care utilising evidence and research. To manage and prioritise a specialist caseload, including those with multi-pathology and complex needs. Duties will include assessment, using standardised and none standardised assessment tools, diagnosis, treatments and interventions as part of an overall care plan, and discharge planning working collaboratively with the multidisciplinary team. To provide a specialist response in complex and highly complex cases, to plan and implement patient centred individual or group intervention. To monitor, evaluate, analyse and modify treatment to measure progress and demonstrate clinical effectiveness of intervention using evidence-based practice and outcome measures. To utilise data and information from a range of sources to identify patient/service user condition to guide decision making. Within field of expertise and policy and where appropriate, request specific clinical tests to establish a diagnosis. To undertake the role of health promoter and educator for patients/service users and their support networks. To ensure that professional standards are upheld by the staff members working within the postholders sphere of responsibility. To manage all incidents as per policy and participate in risk management and Quality and Safety processes e.g SWARM. To be accountable for ensuring that safeguards are in place for patients/service users and escalate any safety, safeguarding or risk concerns following the appropriate processes.
