Crynodeb
Key Areas of Responsibility To work alongside the multi-disciplinary team across the PCN to assess and triage patients, including same day triage. Provide definitive treatment such as the prescription of medications in accordance with guidelines (locally and nationally) and make referrals to other members of the primary care team. Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action. Undertake complex decision making to inform the diagnoses, investigation, and complete the management of episodes of care within a broad scope of practice.Proactively take a personalised care approach and population centred care approach to enable shared decision making with the presenting patient.
