Are you a Registered General Nurse or Paramedic looking for your next challenge? We have an exciting opportunity to join the East Berkshire Urgent Community Response (UCR) and Virtual Ward Team on a 12-month fixed term contract.

This role involves providing clinical assessment and care to patients in their own homes and care homes, supporting those who require a rapid response to help prevent unnecessary hospital admissions. Working as part of a multidisciplinary team, you will deliver high-quality, patient-centred care while supporting patients to remain safely at home.

You will also provide leadership within the team, including line management, clinical supervision, and support for junior nurses and support workers, while helping maintain high professional standards across the service.

Applicants must be a Registered General Nurse or HCPC-registered Paramedic with advanced history taking and physical assessment skills or be working towards this qualification. A Non-Medical Prescribing qualification, or willingness to work towards it, is also required. You should be confident working autonomously, able to work under pressure, and have excellent organisational, communication, and time-management skills to effectively prioritise workload.

The post is 37.5 hours per week, working across a 7-day service between 8am and 8pm, including weekends.


• To practice as an advanced level clinician, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service and Virtual Ward in collaboration with the Community Geriatrician and GP.

• As an experienced practitioner, you will take responsibility for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.

• The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner

• To provide leadership and line management to the Band 6 clinicians and Band 3 & 4 healthcare/multi-therapy assistants.

• Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC/HCPC, information governance and local / national guidance.

• Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery.

• Work flexibly in response to service need across our multiple sites
