We have a fantastic opportunity for a highly-motivated and enthusiastic Care Co-ordinator to join Team Up within Erewash.
The care co-ordinator will be part of Team Up supporting the Frailty Service who are responsible for managing care of housebound patients and Care Home Residents. The care co-ordinator is a pivotal role to the Service and will be the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.
The role will also include working closely with the aligned GP Surgeries and Care Homes within Erewash.
The successful applicants will have excellent communication skills, be able to work independently and be confident using IT systems.

Key aspects of the role will involve:
• Be point of contact for care homes staff and patients/relatives.
• Liaising with health care professionals, including GP, Advanced Specialist Practitioner/Community Specialist Practitioner, District Nurse, Therapies and other members of the health care team.
• Referring patients to the voluntary sector as well as the health sector for additional support in and around their home and making telephone contact with patients; acting as liaison between the patient, health professionals and the social sector.
• The Care Co-ordinator will have a holistic approach towards supporting patients within the community, helping them live well at home, working alongside an aligned GP surgery.
• The Care Co-ordinator will contribute to tackling health inequalities in health and social care particularly regarding individuals with long-term conditions. An ethos of promotion of independence and partnership working is integral to this post.
