The Health Squad seeks to support people who have been identified as struggling to access healthcare services, looking at key health inclusion groups to support with health service navigation. The role involves delivering health assessments across a range of settings to a diverse range of cohorts including those affected by homelessness, substance misuse and GRT for example. (including clinically relevant tests for which full training will be provided). For those with higher levels of needs, the successful candidate will work to signpost, refer and advocate on their behalf about NHS navigation. This can involve speaking with key stakeholders such as GP and specialist service providers.
We are looking to appoint someone skilled in:
• Effective client support & relationship building
• Working as part of an effective outreach team
• Effective caseload management, including dynamic risk assessment and using a person-centred approach
• Identifying support pathways through asset based, holistic methodologies
• Approaching people and striking up a conversation

Job Purpose

Health Squad will operate across County Durham to support the navigation of people found to be at risk through both generic and specialist healthcare services depending on their priority needs. The aim of the initiative is to support reductions in Health Inequalities linked to the CORE20Plus5 “Plus” programme supporting people with multiple and complex health and healthcare needs. The target groups have been identified as people who are homeless, prison leavers / on probation, vulnerable migrants, gypsy roma and traveller communities, people with learning disabilities, people from ethnic minority communities and carers.
The role will be at the forefront of addressing health inequalities across County Durham working in partnership with a variety of other key teams will be vital to the success of the programme.

The purpose of the role is to support the Health Squad Project Lead in the development, mobilisation and delivery of the County Durham Health Squad project and line manage a team of Advanced Practitioners. The post holder will support management, line manage and deliver interventions, including health checks, with clients.

The post holder will use a population health management approach to ensure targeted and prioritised support protecting the most vulnerable populations and improving health and wellbeing across County Durham.
