Job Description Practice NurseJob Purpose To support the clinical team in delivering high-quality patient care by providing assessment, treatment, screening, preventative care and health education to registered patients. The post holder will work collaboratively with the multidisciplinary team to deliver safe, effective and patient-centred care in accordance with practice protocols and national clinical guidance. Special Requirements Registered Nurse (NMC Registered) Evidence of continued professional development Ability to prioritise and organise workload effectively Excellent communication and interpersonal skills Ability to work both independently and as part of a multidisciplinary team Commitment to maintaining high standards of patient care Main Duties and ResponsibilitiesClinical Duties The Practice Nurse will: Assess patient needs and provide appropriate nursing care either independently or in conjunction with a GP or other healthcare professional Deliver treatment room services including wound care, venepuncture, ECGs and other clinical procedures Undertake immunisations and vaccinations Assist with the management of patients with long-term conditions Carry out screening programmes including cervical cytology Provide health promotion and lifestyle advice to patients Recognise and respond appropriately to common childhood illnesses Evaluate the outcomes of care and adapt care plans accordingly Maintain accurate and contemporaneous clinical records Triage and Patient Assessment The post holder will participate in protocol-led triage, including telephone and face-to-face assessment of patients, providing clinical advice or arranging appropriate follow-up with other healthcare professionals where required. Treatment Room Responsibilities Maintain treatment room equipment and supplies Monitor stock levels and report shortages Ensure equipment is maintained and used safely in accordance with practice procedures Investigations and Specimens Collect pathological specimens including blood samples, swabs and smears Undertake investigations requested by GPs in accordance with clinical protocols Administrative and Professional Responsibilities The post holder will: Maintain accurate electronic patient records Complete all documentation associated with patient care Contribute to data collection and reporting requirements Attend practice meetings and participate in multidisciplinary discussions Support the development of practice policies and clinical standards Education, Training and Research The Practice Nurse will: Participate in the teaching and support of students and new staff Contribute to the induction of new clinical staff Participate in relevant research projects where appropriate Maintain professional development through training, study days and CPD Liaison Maintain effective working relationships with other healthcare professionals, community services and external agencies involved in patient care. Confidentiality The post holder will have access to confidential information relating to patients, staff and the practice. All such information must be treated as strictly confidential and handled in accordance with data protection legislation and practice policies.
