An exciting opportunity has arisen for a nurse associate to join our children’s community complex care team.

We are looking for a nurse associate who is hard working, reliable, trustworthy, conscientious, caring and will embrace the challenge of caring for children with complex medical needs within the family home and community settings. Full training will be given to meet the individual needs of the role and patients. You must be flexible and able to work both office hours and support and cover shifts over 7 days a week, 24 hours a day within homes, school and occasionally hospital. The shift times are individual to each package of care, however the whole of your shift will be spent with one client, so you will get the opportunity to really make a difference.

You will support the nurses in the planning and delivery of care within the community. You will provide training to the nursing assistant’s and there will be opportunities for education within the wider community.

The complex care team is part of an integrated team which also incorporates children’s community palliative care and special needs school nursing. Therefore, you may on occasions also provide care at end of life or other community locations.

As this is a community citywide post, you must possess a driving license and have access to a vehicle.

Under indirect supervision and working in partnership with a Registered Nurse provide and deliver a high standard of nursing care, contributing to the ongoing assessment, providing and monitoring evidence based care and working in partnership with the multi-disciplinary team.

Provide compassionate, safe and effective care and support to children in a range of care settings.

Monitor the condition and health needs of patients within their care on a continual basis in partnership with patients, families and carers, ensuring appropriate escalation when required.

Care for a patient’s nursing needs whilst promoting comfort and well-being, ensuring their personal and social needs are met.

Safely undertake care interventions to meet the patient’s nursing needs for which you have attained competence.

Prioritise and manage own workload, recognising where elements of care can safely be delegated to other colleagues, carers and family members.

Provide and receive sensitive information concerning a patient’s medical condition.

Work with the Registered Nurse and engage with other health and social agencies to ensure a safe, individualised approach to the care of the child/young person.

Maintain accurate, contemporaneous patient records in line with Trust policy and the NMC Code.

Be responsible for stock checks, stock rotation and ordering and equipment servicing.
