Due to promotion of the current post holder we are seeking an enthusiastic Rehabilitation Assistant to join our Integrated Community Rehab Service. This is an excellent opportunity for those passionate about improving the lives of older people, and with a clear vision of the role with a commitment to rehabilitation.

Although based in one service there will be opportunities to work with a variety of services that make up the ICRS. These services include Falls, Community Physio, Occupational Therapy, Orthopaedics and Post Covid.

All rehabilitation is carried out in people's homes. Therefore applicants MUST have a UK driving license and access to a vehicle to travel across central Manchester / have other means to be able to carry out home visits. Please note that evidence of this will be required. If you are unable to provide this evidence you will not progress to interview.

The successful applicant will work with a team of Physiotherapists and Occupational Therapist as well as the wider Multi-Disciplinary Team to provide treatment and intervention as prescribed by a registered clinician.

Applicants should have a keen interest in supporting the multi professional team of specialist Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Assistants, in addition to the wider objectives of the Integrated Community Rehab Services.

The postholder will initially work as a member of the team providing rehabilitation for patients who have fallen and to reduce the risks of future falls. The team is multi-professional and provide assessment and rehabilitation at home with the aim to maximise function and independence of patients in their own homes and their community.

The post holder will follow agreed protocols for delivering home exercises and reviewing equipment, ensure appropriate and timely input, and following pathways of care. They will communicate with all agencies working with the service user whilst respecting patient choice and autonomy. They will undertake a wide range of tasks both in the home and other community settings.
