Join Our Team as a Dementia Support Coordinator!
Do you want to make a meaningful difference to people affected by dementia? Are you compassionate, organised and committed to empowering people to live their best possible lives?
As a Dementia Support Coordinator, you’ll provide high high-quality information, advice and community-based support across Calderdale. You’ll manage referrals, arrange consultations and work directly with adults living with dementia – as well as their families and carers – to help them remain independent and connected.
You’ll co-produce personalised plans with clients, record outcomes and work within a trauma-informed, person-centred approach. Building strong relationships with partners and community organisations will be essential, alongside leading on a designed champion area to drive innovation and continuous improvement within the Service.
Who we’re looking for:
• Knowledge & understanding of living with dementia from both the client and carer/family perspective
• Understanding of barriers individuals affected by dementia face and ability to help identify and overcome them
• Experience of managing a client caseload, providing advice, guidance and signposting.
