What will you be doing?
We are looking for a Senior Social Care Assessor to help us provide community-based Adult Social Care within Herefordshire. As a Senior Social Care Assessor, you will work as part of a wider team of occupational therapists, partners, social care assessors and social workers with scope to practice with a diverse group of adults and informal carers, undertaking strengths-based assessments, mental capacity assessments, care management, and reviews.
About You
As a Senior Social Care Assessor, you will have:
� An NVQ3 qualification or above in related field, or equivalent qualification/experience. GCSE or Equivalent in English and Math�s.
� Experience of working in a care environment.
� An awareness and understanding of current legislation and guidance and their impact on adult social care profession.
� Knowledge of undertaking person centred, strengths based, outcome focused, assessment, care management and review.
� Demonstrable experience of supporting people to identify outcomes that they would like to achieve and supporting them to develop a strengths-based plan of how they will work towards achieving them.
� An ability to develop and maintain good working relationships with service users, their families, carers and advocates, colleagues and external organisations.
