Crynodeb
You will act as a first point of contact for patients and external agencies, responding to enquiries professionally and sensitively, whether in person, by telephone or electronically. You will gather relevant information, care navigate in line with practice protocols, and ensure patients are directed to the most appropriate clinician or service. You will also support home visit requests and liaise with community and secondary care providers where required. You will manage appointment systems effectively, including booking, amending and cancelling appointments, processing recalls and supporting demand management processes. This includes recording accurate information on the clinical system, supporting long-term condition monitoring and ensuring patients receive timely follow-up. The role also includes general administrative duties such as processing incoming and outgoing correspondence, scanning and coding documents, maintaining accurate patient records, and providing clerical support to the wider team. You will contribute to maintaining smooth day-to-day operations within the practice and undertake mandatory training as required.
