We are seeking a dedicated and compassionate Adults Social Worker to join our Locality team at Grade H. In this vital role, you will be responsible for ensuring the wellbeing and safety of adults within our community. You will work collaboratively with individuals, families, and other professionals to provide support and guidance tailored to the unique needs of each client. Your expertise will help empower adults to lead fulfilling lives, while navigating the complexities of social services. With a focus on promoting independence and enhancing quality of life, you will conduct assessments, develop care plans, and coordinate resources to meet the diverse needs of our clients. The role requires a strong understanding of relevant legislation and best practices in social work, along with excellent communication and interpersonal skills. You will also engage in community outreach and partnership building to ensure access to needed services. Responsibilities
Conduct assessments of individuals' needs and risks
Develop and implement care plans tailored to clients' unique situations
Provide support and guidance to clients and their families
Coordinate with healthcare and community resources to provide comprehensive services
Advocate for clients' rights and access to services
Maintain accurate and up-to-date documentation of client interactions and progress
Participate in multidisciplinary team meetings to discuss case management strategies
Requirements
A recognized qualification in Social Work (Degree or equivalent)
Valid social work license or registration
Experience working with adults in a social work setting
Strong understanding of relevant legislation and policies
Exceptional communication and interpersonal skills
Ability to work independently and as part of a team
Empathy and a non-judgmental attitude towards clients
Hours Per Week:
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £23.46
