We are excited to offer a new opportunity for an Advanced Practice Frailty Occupational Therapist to join an innovative Primary Care Network in Woodberry Wetlands. This post will be based across 4 GP practices and will be hosted by the Adult Community Rehabilitation Team at Homerton Healthcare.
This forward thinking role is ideal for an experienced OT passionate about proactive care, prevention, supporting clients with rising needs and community-based frailty rehabilitation.
You will work closely with Primary Care colleagues and other health/voluntary community services to support the shift of care in the community and contribute to the prevention agenda by offering advice, signposting, assessment and intervention. You will deliver holistic, person-centred care to support individuals functional independence at home and in their local community.
We are looking for an experienced, values‑driven Occupational Therapist who can demonstrate:
• Advanced clinical skills in frailty, rehabilitation and case management.
• Strong leadership, communication, and partnership working abilities.
• A passion for prevention, proactive care, and reducing health inequalities.
• Confidence working autonomously in a dynamic primary care environment.
• Commitment to continuous improvement and service development.

Some of the main duties are as follows:
• Work at an advanced level within the four pillars of advanced clinical practice: clinical practice, leadership and management, education, and research.
• Develop and shape the OT role to meet local population needs in consultation with PCN leads and Homerton ACRT.
• Deliver proactive, holistic occupational therapy for individuals with frailty or rising need, supporting early intervention and prevention in line with neighbourhood priorities.
• Provide specialist assessment, rehabilitation, and intervention to the PCN population, exercising independence of judgement and advanced clinical reasoning.
• Lead and contribute to service improvement, innovation, and evaluation, embedding sustainable quality through training, supervision, research, and continuous improvement.
• Collaborate as part of an integrated, multidisciplinary team, addressing the impact of physical, social, psychological, environmental, and mental health issues on functional independence.
• Act as a clinical role model, delivering care responsive to changing requirements and informed by local population health needs.
• Provide expert advice, guidance, and teaching for patients, carers, and the multidisciplinary team, ensuring consistent, high-quality care.
The Adult Community Rehabilitation Team is a multi-professional team offering rehab and condition management to clients living in C&H with long term physical and/or neurological conditions.
