To actively embed any recommendations following serious case reviews or SUI ensuring alllearning is acted upon To undertake the accurate collection of data for use in service reporting, audit and formonitoring the Service level agreement. To update therapy time and outcome measures inline with Stroke Sentinel National Audit Programme (SSNAP) on patient-based system datain a timely fashion, fully complying with data quality requirements. Clinical Responsibilities Contributing to the planning, co-ordinating, delivering and evaluatingthe Community stroke service within North Tees and Hartlepool NHS Foundation Trust Responsible for the assessment, diagnosis, accurate prognosis, planning, implementationand evaluation of care as an autonomous and lone working practitioner to clients withcomplex cases where clinical reasoning, evidence based practice and expertise areessential to develop effective individual therapy plans. To be a source of knowledge in the specialist areas for the team, wider health carecommunity and multi-agency partners providing specialised advice and clinical practice. Continue to develop and suggest new and innovative ways of working based on availableevidence To have highly developed effective communication skills, both verbal and non-verbalincluding barriers to communication. To communicate information that may be sensitive or distressing appropriately. To be aware of own scope of practice and the scope of other professionals and referappropriately to provide holistic care. To have specialist theoretical knowledge and applied clinical practice in the specialist area To work in a coordinated, multidisciplinary manner To be involved in health promotion initiatives To monitor the health and wellbeing of people with complex and changing needs across theservice, assisting in protecting individuals and groups whose health and well being may beat risk. To be responsible for maintaining accurate comprehensive patient treatment records in linewith Trust policies and to communicate assessment and treatment results to the appropriatedisciplines in the form of reports and letters. Represent physiotherapy service and/or individual patients at the ICSS multi-disciplinaryteam meetings, to ensure delivery of a co-ordinated multidisciplinary service, and integratephysiotherapy treatment into the treatment programme. This will include discussion ofpatient care, patient progress and involvement in discharge planning. Provide specialist clinical advice to others regarding the management and care of strokepatients.
