Summary
• As part of the Integrated Community Stroke Service (ICSS) this post is trust wide across all
areas of the Stroke therapy service including the acute ward to provide an acute 7 day
service.
• Providing care to a diverse population, to deliver a high standard of physiotherapy care to
stroke patients and their carers in the community.
• To perform comprehensive physiotherapeutic assessment of patients with diverse
presentations and complex physical and psychological conditions following a stroke.
• To asses stroke patients to develop and deliver an individualised treatment programme
specific to individual needs.
• To hold responsibility for own caseload and, working without direct supervision but together
with the other members of the MDT, deliver a comprehensive package of care to stroke
patients and their carers.
• Supervision takes the form of supervision sessions, in service training, peer support groups.
• Access to advice and support from a more senior physiotherapist is available when required.
• To undertake all aspects of clinical duties as an autonomous practitioner.
• To contribute and be involved in service changes/developments through quality improvement
projects.
• To provide cover for other physiotherapists in the service as required and to cover weekend
working on the acute ward.
• Must have UK driving license and access to a car.
• To work autonomously to complete advanced assessments and treatment of patients within the speciality who may have complex presentations and to determine Physiotherapy treatment indicated and to maintain records as an autonomous practitioner.
• To act as a specialist resource to other professionals for the management of patients following a stroke.
• To represent the Physiotherapy Service at the multi-disciplinary team meetings to ensure the delivery of a co-ordinated multi-disciplinary service and integrate Physiotherapy treatment into
the overall management programme.
• On a daily basis to implement decisions on allocation of limited resources which directly affect
level of physiotherapy intervention to specific patients and have to justify those decisions to
patients, carers and others.
• To undertake therapeutic manual handling in compliance with Trust manual handling policies
and local guidance at all times.
• To deal sensitively with patients and carers who have high levels of anxiety and aggression
E.g. caused by loss of mobility and independence, cognitive impairment e.g. dementia or
learning difficulties.
• To interact with patients through an interpreter when English is not the patient’s first
language.

• Identify personal training and development needs and achieve annual personal and
departmental objectives through SDR process and provide evidence.
• Participate in and log all CPD activities.
• To actively embed any recommendations following serious case reviews or SUI ensuring all
learning is acted upon
• To undertake the accurate collection of data for use in service reporting, audit and for
monitoring the Service level agreement. To update therapy time and outcome measures in
line with Stroke Sentinel National Audit Programme (SSNAP) on patient-based system data
in a timely fashion, fully complying with data quality requirements.
• Clinical Responsibilities Contributing to the planning, co-ordinating, delivering and evaluating
the Community stroke service within North Tees and Hartlepool NHS Foundation Trust
• Responsible for the assessment, diagnosis, accurate prognosis, planning, implementation
and evaluation of care as an autonomous and lone working practitioner to clients with
complex cases where clinical reasoning, evidence based practice and expertise are
essential to develop effective individual therapy plans.
• To be a source of knowledge in the specialist areas for the team, wider health care
community and multi-agency partners providing specialised advice and clinical practice.
• Continue to develop and suggest new and innovative ways of working based on available
evidence
• To have highly developed effective communication skills, both verbal and non-verbal
including barriers to communication.
• To communicate information that may be sensitive or distressing appropriately.
• To be aware of own scope of practice and the scope of other professionals and refer
appropriately to provide holistic care.
• To seek advice from highly specialist therapists for complex cases
• To observe and maintain the Physiotherapy minimum standards of clinical practice and
HCPC code of conduct.

• Follow the Trust’s hospital infection prevention and control (HIC) policies and procedures to
protect patients, staff and visitors from healthcare- associated infections.
• Performs the correct hand hygiene procedures, when entering and leaving clinical areas.
• It is imperative that the post holder is a team player, but will also be expected to work under
their own initiative.
• To be actively involved in the clinical supervision process.
• To have specialist theoretical knowledge and applied clinical practice in the specialist area
• To work in a coordinated, multidisciplinary manner
• To be involved in health promotion initiatives
• To monitor the health and wellbeing of people with complex and changing needs across the
service, assisting in protecting individuals and groups whose health and well being may be
at risk.
• To be responsible for maintaining accurate comprehensive patient treatment records in line
with Trust policies and to communicate assessment and treatment results to the appropriate
disciplines in the form of reports and letters.
• Represent physiotherapy service and/or individual patients at the ICSS multi-disciplinary
team meetings, to ensure delivery of a co-ordinated multidisciplinary service, and integrate
physiotherapy treatment into the treatment programme. This will include discussion of
patient care, patient progress and involvement in discharge planning.
• Provide specialist clinical advice to others regarding the management and care of stroke
patients.
