An exciting opportunity has arisen to join the Care Brokerage Team which sits within the Urgent and Emergency Care and New Models of Care portfolio. The Brokerage Officer position is a FTE (37 hours per week) post, operating Monday to Friday.

About the Role:
To manage referrals for Adult Social Care and Health services via a single point of access, ensuring referrals include all relevant information to inform providers accurately. This also involves discussions with clients, carers and partner organisations.
Develop and maintain relationships with providers and become familiar with services in order to match clients to appropriate services.
Develop and maintain relationships with referral sources and to raise awareness of referral processes and available support and to act as the conduit between referral sources and providers.
Identify and make referrals to the most appropriate providers based on the information gathered from the referral.
Monitor and assess the progress of referrals, liaising with providers, responding to calls, emails and queries and ensuring all systems are updated in a timely manner.
To facilitate timely hospital discharges to support system flow.

About you:
The successful candidate will demonstrate:
• Experience of working within the Care Sector
• Experience of working with Health would be an advantage.
• The ability to work under minimal supervision in a fast paced and high-pressure environment.
• The ability to be flexible to the demands of the service.
• Excellent relationship management skills
• Excellent IT skills
