Summary

The Transitions service works with people aged 17-25 to support people to move into adult services. This will include people are part of the transitional safeguarding cohort who require additional support to move into adult services. Young people will remain with childrens social care until they turn 18. The people open to the transitions team will have a named worker. The transitions service staff will work alongside the childrens worker to build a relationship with the young person and ensure that there is smooth handover between services. The Transitions service can start to assess people under adult social care legislation and plan for what support is needed so that when they turn 18 the appropriate support will be in place When people turn 25, they will move into the appropriate adult social care team should they still require ongoing support. This will require close liaison between the transitions service and adult social care. Specific duties and responsibilities include: To undertake Care Act Assessment use strength-based and person-centred social work approaches with customers and to plan for the provision of support and/or services with customers in order to assist them meet their outcomes and to live as independently as possible fulfilling their individual potential. To be the primary worker with case responsibility for people who are allocated to the Transitions service, promoting independence and ensuring that the eligible Care Act needs of individuals are met. Take an active role in the team, being open to constructive feedback and continuing professional development. Take responsibility, alongside the team, for regularly reviewing and revising plans to ensure promoting independence and least restrictive practices are being implemented successfully. Support the team to establish and build good relationships across directorates and with partners. Liaise effectively with social care practitioners and transitions co-ordinator to ensure a joined-up approach. To be responsible for the day-to-day interpretation of and implementation of statutory duties and departmental policy and procedures in the provision of services to children, young people and vulnerable adults. To carry out regular reviews of individual plans, record outcomes and formulate revised plans which are outcome focused. Provide support and guidance to the wider team on case management, promoting least restrictive approaches. Professional supervision, training and effective relationship development with Level 2 social workers and social care practitioners and transitions co-ordinator. To work proactively and in partnership with colleagues and other agencies to plan and deliver coordinated services which deliver high quality outcomes. Utilise understanding of individual needs to devise and propose innovative solutions to ensure needs are met. To ensure accurate and timely recording of all information /assessment activity onto the Social Care records database and Transitions database. To have a well-developed understanding of assessment and the management of risks for individuals and to be able to apply Council policy and support customers to manage risk appropriately. To identify where children or vulnerable adults may be at risk and to act in accordance with the Departments Safeguarding policies and procedures. To gather sufficient information about customers in order to be able to accurately assess their needs against eligibility criteria. To ensure that local knowledge about universally available services is up to date including what is available within the voluntary and community sector, as well as the Departments commissioned and contracted services and to signpost/advocate for customers accordingly. To be an ambassador for the Council and the department at all times, always representing the Council positively, professionally and appropriately at meetings with customers, external partners and agencies.
