Crynodeb
The post holder will embrace Lifestyle Medicine and the approach to Chronic Disease management and Mental Illness and help patients to create their own Lifestyle Prescription to compliment and support their usual medical care. The Health Coach will also work with healthy individuals to help them adopt daily preventative measures including stress management, exercise, and healthy eating habits. The post holder will support, educate & motivate the patient to find simple ways to better health, break through resistance and overcome self-sabotage. Operational/ Main Responsibilities Support patients to make effective, achievable, and sustainable health and fitness behaviour change. Work with patients both in a group and individually (in person, virtually, or over the phone) to discuss goals, progress, and challenges. Provide patient support, education, coaching and advice, whether they have a chronic disease or are healthy, to improve their overall health condition. Understand patients prevailing health conditions Identify the need for behavioural change to improve health status, reduce health risks and improve quality of life. Examine and record the patient specific observations, weight, height, etc., accurately. Identify what stage of readiness to change the patient is currently in, with appropriate and timely use of motivational interviewing, coaching and positive psychology techniques to: a) help the patient move forward toward taking positive action/ steps and/or b) maintain their current healthy behaviours Apply knowledge of, diet, nutrition, or relevant methods to suggest solutions. Suggest exercises, diet charts or mental stress reducing techniques. Deal with the patient with a positive and empathetic attitude to make them feel comfortable Strategic To support the PCN with the development of Health Coaches. Support and deliver health inequalities work with other members of the care coordination and practice teams, to address the wider determinants of health. Leadership To be able to motivate and work with patients and/ or clients to support the wider clinical team with any patients that they feel need an intervention from a health coach. To provide expert advice and guidance to other members of the PCN in relation to Health Coaching. Governance Produce complete and accurate records of patient consultation, in line with best practice, confidentiality, policies and procedures Deliver care according to NHS guidance, NICE guidelines, and evidence-based care Take part in the maintenance of quality governance systems and processes across the PCN and its activities. Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required. Work with other clinical teams on improving the quality of healthcare in response to local and national policies and initiatives as appropriate Evaluate patients response to health care provision and the effectiveness of care Support and participate in shared learning across the PCN and wider organisation Manage, review, and identify learning from patient complaints, clinical incidents, and near-miss events Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them Ensure compliance with policies, procedures and guidelines for self and others, by acting or alerting senior management team if the practice appears to contravene policy, or if there are concerns over any aspect of patient care. Health and Safety The post-holder will manage their own and others health & safety and infection control as defined in the SEL GP Group Health & Safety Policy, the SEL GP Group Health & Safety Manual, and the SEL GP Group Infection Control Policy and published procedures. Comply with SEL GP Group health and safety policies by following agreed safe working procedures Awareness and compliance with national standards of infection control, hygiene, regulatory / contractual / professional requirements, and good practice guidelines. Reporting incidents using the organisations Incident Reporting System. Confidentiality Maintain confidentiality of information, always acting within the terms of the Data Protection Act and Caldicott guidance on patient confidentiality. Maintain an awareness of the Freedom of Information Act. Equality and Diversity The post-holder will support, promote, and maintain the SEL GP Group Equality & Diversity Policy.
