Staying Steady is delighted to invite applications for a part‑time Band 4 Rehabilitation Assistant to join our friendly, multidisciplinary team within the Milton Keynes Home1st service. We are a key part of the Milton Keynes Falls Pathway, a joint NHS, Council and voluntary sector initiative supporting people who have fallen or are at risk of falling. Our aim is to help individuals understand and manage their falls risks, build confidence, and maintain independence through regular physical activity.

About the Role

As a Band 4 Rehabilitation Assistant, you will work both independently and as part of the wider team. Your responsibilities will include:
• Completing home hazard assessments
• Carrying out exercise and activity assessments
• Supporting multifactorial falls assessments to identify individual risk factors
• Working collaboratively with patients and their support networks to develop personalised plans
• Delivering interventions on a one‑to‑one and group basis

This is a community‑based role that relies on strong communication, empathy, and partnership working skills. You will be supported through regular supervision from therapists and nurses within Staying Steady and will work closely with the wider multidisciplinary team to meet patients’ rehabilitation needs. Previous experience in a physical rehabilitation role is essential.

To undertake specific assessments and provide education and treatment plans to support the physical rehabilitation and education of service users, with the aim to reduce their falls risks and maintain independence and to promote physical activity.

To work with the team to facilitate and manage the Otago group. This will include exercises intervention and basic patient education.

To provide flexible programmes and interventions of therapeutic activities, under the guidance of the Staying Steady registered staff

Undertaking low level equipment assessments and providing equipment.

Main duties:

Carry out assessments and interventions in the community, patients own home, care homes, or supporting in the consultant led Staying Steady clinic.

To provide high standards of documentation and communication, including the use of electronic database i.e. System1

Carry out other duties deemed necessary by Staying Steady professionals.

To provide a support role in assisting the Staying Steady professionals in performing assessments and care planning as required.

To use information gained from assessment to develop a problem list and proposed intervention options. To action Therapy interventions following discussion with the MDT.

To carry out Therapy tasks which may involve moving and handling and potentially personal care.

To effectively use a range of verbal and non-verbal communication tools to engage and negotiate with patients across a multicultural and intellectual spectrum.

Staying Steady is based at the Open University, with work carried out across multiple bases and in patients’ homes or care homes. You will be provided with:

· To undertake specific skilled support work to aid rehabilitation of service users, in achievement of optimal independence with or without the assistance of others

· To provide flexible programmes and interventions of therapeutic activities, independently or under the guidance of staying steady registered staff as required

· Follow treatment plans, developed by health professionals in the service

· Work without direct supervision, monitoring progress and minor adaptations to interventions as required, including patient assessment

· Managing and facilitating with Otago Groups with the other Band 4 rehabilitation assistant, with support from other members of the MDT
