Demonstrate highly developed communication and motivational skills to facilitate patients to manage their condition To Demonstrate the ability to deliver or discuss difficult diagnosis and prognosis with tact, empathy, managing distressed or aggressive patients or carers/relatives. To communicate accurate and sensitive information to patients and carers using an appropriate and accessible medium, taking into consideration any relevant factors such as background, culture, gender, age, and any physical, sensory, or learning difficulties, adopting an empathic all times including when communicating with distressed or aggressive patients. To respect the dignity and preferences of patients and obtain consent to therapy, in some circumstances you would be working in the patients best interests where consent cannot be gained due to capacity. To provide and receive complex and sensitive information, verbally, written and/or electronically, to and from consultants, GPs, patients, colleagues and other agencies. Attend and participate in interdisciplinary meetings and other relevant meetings both within CWPT and UHCW as required To have a broad knowledge of voluntary and independent organisations relevant to the specialist area and establish effective relationships with local services in order to enhance the delivery and transfer of care. Attend staff meeting as required Ensure all relevant information about the patients treatment or progress is communicated to other team members involved and other agencies as required Communicate with patients and relatives/carers the reasoning for assessments and interventions and establish full understanding and consent prior to delivery Employ and utilise formal and non-formal forms of assisted communication with patients or relatives/carers with communication impairments Communicate well with other team members ensuring accurate understanding of delegated therapy programmes to establish safe individual practice alone in the community Ensure relevant communication from the team is passed to Lead Therapists as required. To work Collaboratively with other key partnership organisations to ensure effective, safe delivery and transfer of care. To be consulted as a source of information by MDT members seeking Advice/information.
