About the team

This team works closely with the multidisciplinary team to complete care act assessments, mental capacity assessments, and strengths-based care planning for adults preparing to leave hospital. You will coordinate discharge arrangements, D2A pathways and make sure that patients receive timely discharges. The team's ability to manage a fast paced, everchanging caseload is key to the success the team has had.

A valid UK driving licence and car is not essential but desirable for the role.
