Crynodeb
This role involves managing a complex caseload, completing strengthsbased assessments, and creating reablement plans that promote independence. You will assess and manage risk, coordinate each customers reablement journey, and work closely with families, carers, and health and social care partners. Responsibilities include moving and handling assessments, recommending home adaptations, supporting carers, and ensuring compliance with legislation and Council policies. You will maintain accurate records, engage in supervision and professional development, and contribute to team learning. The role also requires staff supervision, safeguarding involvement, daily travel, occasional outofhours work, participation in civil emergencies, and decisions on Blue and Bus Badge appeals.
