As a Health Care Assistant on Pyrland Ward, you will play an essential role in supporting older adults with complex mental health needs, including dementia and acute functional illnesses. Patient Care and Therapeutic Support Deliver delegated care tasks under the supervision of registered nurses, ensuring respect, privacy and dignity. Build trusting therapeutic relationships, offering emotional support during periods of distress. Support patients to engage in meaningful and therapeutic activities, both individually and in groups. Escort patients safely, offering reassurance and maintaining dignity during leave or off ward activities. Assist with physical healthcare tasks such as continence care, wound care and patient observations. Use de-escalation techniques when supporting individuals experiencing agitation or challenging behaviour, adopting least restrictive approaches and putting safety first. Communication and Collaboration Communicate clearly and compassionately with patients, families, carers and the wider MDT. Support a positive ward culture through teamwork, inclusion and shared responsibility. Welcome and assist visitors sensitively, ensuring confidentiality and adherence to ward procedures. Documentation and Information Management Record observations, interventions and care activities accurately on paper and electronic systems (RiO). Report incidents, accidents and concerns in line with Trust policy. Handle information sensitively, adhering to confidentiality, data protection and safeguarding requirements. Ward Environment and Resources Assist with maintaining a safe, clean and well organised ward environment. Use equipment appropriately and escalate faults or hazards promptly. Support infection prevention and control standards at all times. Professional Development Participate in mandatory training, clinical supervision and appraisal. Act as a positive role model for new staff and students, supporting a learning focused environment. Contribute to quality improvement initiatives and ongoing service development.
