Crynodeb
As Practice Clinical Pharmacist, you will: - Work as part of a multidisciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas - Provide pharmaceutical consultations to patients with long term conditions as an integral member of the team - Offer proactive leadership on medicines and prescribing systems to colleagues, patients and carers - Improve patient and carer understanding of and confidence in their medication, supporting safe and effective use - Provide clear and timely advice in response to medication related queries from patients and staff - Organise and oversee the practice medicines optimisation systems, including repeat prescribing and medication review processes - Promote high-quality, cost-effective prescribing in primary care, including Structured Medication Reviews and Enhanced Health in Care Homes - Improve prescribing quality and effectiveness through clinical audit and education, supporting performance against NICE standards and prescribing guidance - Implement changes to medicines following MHRA alerts, product withdrawals and national guidance - Contribute to a positive patient safety culture through reporting and investigation of medicines incidents - Review hospital discharge notifications and reconcile medication against current repeat prescriptions - Support prescribing budgets through ongoing monitoring, audit and analysis of drug use
