Are you an experienced practitioner who would like to work within a community based MDT, having a passion for supporting patient care and keen to make a difference? We are looking for an enthusiastic Clinical Specialist Practitionerwho wants to join an established admission avoidance service to support patients to stay at home where possible, reducing the need for hospital admission.

The Clinical Practitioner will work primarily in our Urgent Care Crisis Response service but will have the opportunity to support the Hospital at Home service. They will provide clinical assessments at home working closely with our ACPs. We receive referrals from community services, NWAS, 111, GPs and facilitate ED deflections with a focus on keeping patients safe at home where possible.

As a confident communicator with strong organisational skills, you will thrive in an MDT environment and will be instrumental in supporting and developing this service, to ensure a high-quality service for our patients. With a passion for continuous development and service improvement, you will be involved in projects and trust initiatives. You will receive regular supervision, in-service training, and support.

Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.

Assess, plan, prioritise, implement and evaluate interventions to meet patients’ needs.

Contribute to the organisation of multi-disciplinary, patient-centred care.

Facilitate patients, relatives and carers to take an active role in care given, providing information as appropriate to remain safe at home.

Using your experience and knowledge, you will support other team members to ensure we deliver a high-quality service for our patients, providing education and support as needed.

You will also have the opportunity to work across hospital and community services whilst developing key links within primary care and the wider networks within acute and community.
