The Role
Do you want to be part of a new initiative within our bed-based intermediate care services? These new posts will support the multi-disciplinary team (MDT) with discharge planning and communication, supporting in the breakdown of barriers and providing continuity with communication for service users and their families.
These posts will support the MDT in completing referrals into other services, to support timely discharges. A comprehensive induction and training programme will be provided on succession into the role, however as these are new posts, the post-holders will have the opportunity to support with the implementation and development of this role.
About You
You must have experience of working in Adult Health or Social Care and have completed qualifications within this area.
You should be confident at communicating with a range of people, including professionals, service users and their families.
You should be competent with using a computer, including Microsoft Office and case management systems, and be able to complete referrals and maintain accurate records.
This post requires the post-holder to work within a multi-disciplinary team. The postholder will be responsible for planning, prioritising and managing their own workload, therefore you must be organised and willing to work independently.
You must ensure that the service is delivered to the highest achievable professional standards to support service users to be a part of their discharge planning, to promote person centred care.
