The Role
Join Bolton’s Adult South Neighbourhood team, working closely with the multi-disciplinary team to support adults and carers eligible for assessment and care under the Care Act 2014.
We focus on supporting people to live well in their own home through integrated working with Social Workers, Occupational Therapists and Nurses to deliver person-centred and strength-based support to protect the vulnerable and build strong, supportive communities.
Key duties include conducting care act assessments, support planning, reviews, mental capacity assessments, participating in best interests’ meetings, and handling complex cases. The role also involves safeguarding, risk assessment, and coordinating resources from social care, health, and other agencies.
About You
As a social worker, your role is key in ensuring assessed needs and the wellbeing of those whom we support are managed in a safe and inclusive way.
You will adopt a strength-based approach to your practice and seek to provide a person-centred service. You will be a resilient practitioner who can manage competing demands and prioritise work effectively. You will be a confident decision maker, who can stay calm under pressure.
While in this role you will need to demonstrate passion, commitment and enthusiasm to working closely with those with care and support needs, their families and carers, other professionals and your managers and peers.
You must hold a degree in social work, be registered with Social Work England, with evidence of continuing professional development. You will possess a strong understanding of the application of the Care Act 2014, Mental Capacity Act 2005 and the principles and duties underpinning safeguarding adults.
