To assist in the delivery of high-quality nursing care as part of the Continence Service, to the registered population of Greenwich within patients’ homes, including care homes.

To support the Continence Specialist Nurses within clinic setting.
• To perform continence reviews within the community, updating product orders as appropriate and manage the review caseload efficiently.
• To perform and record a simple patient continence assessment review to determine any change since the initial assessment: this will involve taking a patient history; examining patients’ skin which may require some manual handling; reviewing and providing advice on pad/product usage and application
• To communicate patient visits daily to the nurse in charge.
• To enter clinical activity related to patient contacts into the electronic computer record system (RiO).

Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. We are the largest NHS provider of prison health services providing healthcare to prisons within Devon, Dorset, Bristol, Wiltshire and Gloucestershire, Kent and South London. • Undertake routine reviews of patients with bladder and bowel conditions under the guidance of the Continence Nurse.
• Assess and monitor skin condition, particularly in patients at risk of incontinence-associated dermatitis (IAD) or pressure damage. Gather relevant patient history and document changes in symptoms, skin integrity, and continence needs. Identify any concerns or deterioration in a patient’s condition and escalate appropriately to a registered nurse.
• Primarilyclinical, supporting continence assessments, product fitting, ordering supplies, and contributing to service improvement ideas
