The post is based in theIntegrated Learning Disability Services (ILDS) in the London Borough of Hackney.TheILDSis an integrated, multidisciplinary health and social care team providing services to adultswith a learning disability.

This post provides dietetic services to adultswith a learning disability who have compromised nutritional status.The post is a multi-disciplinary, community-based role based in clinics, and inpeople’s homes,including residential and supported living,offering nutrition support to people with dysphagia, enteral feeding,andlowvolumetolerance.

The role will involve working closely with other MDT members across the service, with other Dietitians working in Learning Disabilities acrossthe wider Trust and with key partners including mainstream services and the Acute Trusts.

There will be opportunities to link with and develop professional practice with other Dietitiansacross theTrust.

Clinical supervision will be provided by the Professional Development Lead Dietitian. There will be training and development opportunities for the post-holder.



Provide dietetic input in a community setting for adults diagnosed with a learning disabilitywith a Hackney GP.

Ensure acute dietary assessments, expert advice based on evidence-based practice, and implement patient-centred treatment plans.

Effectively prioritise and review service users with learning disabilitiesrequiringoral orenteralnutrition support, andbe responsible fortheir dietary assessmentand nutritional care planning, includingrelatedcorrespondence with other professionalsand services involved in their care,as well as their carers/ supporters.

Delegate nutrition screening to Health Support Workers.



Ensure that accurate and comprehensive electronic dietetic records of assessment, treatment, progress and discharge summaries are maintained in accordance with legal,trustand councilstandards.

Monitor effectiveness of dietary interventions through goal setting, audit, outcomemeasurementsand evaluation.

Have a thorough understanding of the current legislationand guidancerelating to people with learning disabilities and their health needs.

Clinical

Contribute to effective team working and capacity assessment within the multidisciplinary team for people with learning disabilities.

Offer easy-read and accessible information to communicate highly complexpatient-related information to promote dignity of choiceregardingdecisions affecting nutrition.

Provide complex nutritional counselling to patients and carers to ensure thorough understanding of clinical condition and dietary treatment plans, empathy, negotiation, persuasion, and motivational interviewing skills to gain consentin order tosupportshort and long termbehavioural change.

Effectively prioritise and review service users with learning disability requiring enteral feeding, and be responsible fortheir

dietary assessment and correspondence with other professionals and services involved in their care.

Liaise with local care providers to share information effectively and assess based on medical and social records.

Manage the dietetic service,ensuring the caseloadis monitored, safeandallocated.

Undertake Initial/Single Assessments utilising advanced clinical skills, knowledge and experience to provide a holistic assessment covering the psychological, social and health care needs of people with learning disabilities, including establishing eligibility for support

Use professional judgement to determinea person’spotential Learning Disability diagnosis, in conjunction with colleagues, as part of Eligibility Assessments.

Assessservice users’understandingandability to consent to clinical interventions and the sharing of information with others, in line with the Mental Capacity Act.

Be aware of latest legislation andguidance and learning around people with learningdisabilities e.g.LeDeR, Transforming care, Mental Capacity Act.

Keep up to date with evidence-based practice, applying this to clinical work.

Undertake health promotion work with people with learning disabilities and their carers e.g. healthy eating.

Provide information and support to clients, their familiesand carers to enable access to primary and secondary health services.

Identify,assessand manage potential risks presented to clients,taking action to minimiseand / or prevent risk throughadvice,consultationand referrals to appropriate services.

Identify, raise,adviseand consult on safeguarding concerns in relation to Vulnerable Adults or Children in Need of Protection, within the framework ofcurrent guidelines and legislation.

Contribute toassessments for eligibility of NHS funded

Continuing Healthcare in conjunction with otherprofessionals andmakesubsequentrecommendationsas toeligibility.

Work in a community settingwithout direct supervision, in line with Trust and Council Lone Working Policies.

Work with interpreters and health advocates in order toremove barriers to communication within a multi-cultural environment, thus endeavouring to make health information accessible to all.

Administration

Be professionally and legally accountable and responsible for all aspects of own workload. This includes Health Care Professions Council of standards ofproficiency, Health Care Professional Councils standards of conduct, performance and ethics, The British Dietetic Association professional standards and the British Dietetic Association Code of Conduct

Collect, analyseand provide data related to the learning disability dieteticfunction andworkloads.

Take the lead in team development work when requiredto improve practice, working with colleagues.

Participate on the broader community services dieteticclinical development meetings,participatingin peersupervision with other members of the community dieteticservices within ELFT.

Attend wider Learning Disability Dietetics network events within the BDA and advocate as a leader in the field while representing ELFT.

Management

To lead and/orparticipatein the drawing up and review ofpolicies and procedures.

Promote and encourage positive working as a result ofleading by example.

Lead and support the development of the dietetic service within ILDS,developing links with colleagues across ELFT.

Contribute to and lead professional, clinical andmanagement meetings asrequired.

Lead designated projects within the wider service.

Take a lead role in supervising dietetics students or supportingdietetic student placements.

Human Resources

Workwithin Trustand Councilpoliciesin the course ofdelivering theirinterventions.

Performance and Quality

Regularlyinitiateand undertake clinical audit usingthe audit cycleand present results to promoteevidence-basedpractice.Aiming to systematically review and improve patient care against explicit criteria and standards.

Participate in continuing professional development asidentifiedthrough a performance developmentprogramme, and throughmaintainingcontact andparticipatingin CPD with local dietetics teams.

Plan and prioritise the dietetic case load balancing patientrelated activity and professional demands.

Record and report dietetic activity in accordance withorganisational Key Performance Indicators.

Provide support and supervision for junior staff members.

Holdresponsibility for the Health, Safety and Welfare ofself and others and tocomply at all timeswith therequirementsof the Health and Safety Regulations.

Always ensure confidentiality, only releasingconfidential information obtainedduringemployment to those acting in an official capacityinaccordance withthe provisions of the Data Protection Actand its amendments.

Develop and provide in service training for colleagues,students and new members of staff as part of the servicedevelopment programme.

Arrange andparticipatein teaching & training andsupervision of junior staff and students from other professional backgrounds.

Participate in Research / QI projects undertaken withinthe service and encourage a research / QI basedapproach to care.

Ensure dissemination of research / QI findings on thehealth of people with learning disabilities to others.

Implement research / QI findings in conjunction with othermembers of the service.

Work within the widerteam budgetary constraints, ensuring that anyinterventions delivered are of high quality and value ofmoney.
