Crynodeb
Be able to manage time effectively and identify priorities of care delivery and Falls service Caseload. Able to manage and maintain standards of the Falls Service; led by defined guidelines, protocols, procedures, codes of conduct and followed at own discretion and caseload responsibility. Responding and escalating to changes in the needs and/or environment. Supporting in the planning to ensure effective use of staff and resources. Refer to and work with multidisciplinary teams to develop and support the service user, family and carers; providing specialised advice on complex patient care and management support with mitigation and specialist advice on care and the management whilst optimising safe activity in the delivery of individualised care. Uses numeracy, literacy, digital and technological skills to support safe and effective delivery of care. Carry out systematic and evidence-based assessments to give holistic care, taking into consideration physical, social, cultural, psychological, spiritual, genetic and environmental factors that may influence service users pathway or treatment. Uses interprofessional and interpersonal skills to facilitate the best communication of highly complex, highly sensitive, and highly contentious information to patients, families/ carers, members of the public and workforce; where this may be challenging, unpredictable and potentially hostile. Accepts accountability and responsibility for delivery of care and decisions within clinical practice, ensuring all care is delivered within scope of practice. Maintains accurate, timely and legible records of documentation and care delivered, including how consent is obtained and whether the patient has capacity. Will be able to provide and support staff to deliver safe management in pre-fall assessment and post fall care. Utilise clinical skills seek guidance and evidence-based practice guidelines working within the parameters of the trust Key Performance Indicators and National Audit of patient Falls and NMC codes of conduct are adhered to. Analyses, assesses, plans, implements and evaluates patients that have been reviewed or referred as care needs continuously change, empowering the service user to achieve optimal health through developing and providing highly specialist packages of care and advice. Advocate for patients best interests and respecting their moral values, beliefs, dignity and ethics. Ability to analyse and interpret complex situations; using interpretation and assessment to mitigate risks and/or issues to patient/client clinical need; escalating to senior team when required. Supports the need of the Trust to manage redeployment to support the provision of safe patient care and skill mix when identified by senior leadership teams. Ensuring to work within scope of practice and professional boundaries. Holds a professional responsibility in Safeguarding vulnerable adults, young people and children, inclusive of patients valuables, adhering to Trust policies and protocols at all times Work with various multi professional teams ensuring co creative working on quality improvement work linking with procurement, medical equipment to ensure the availability of up to date equipment and that training to staff is provided.
