Summary


The post holder will be an expert clinical practitioner able to demonstrate specialist clinical competence and knowledge base. With the support of the Lead Nurse , the post holder will lead the Tissue Viability service at the designated work base, whilst providing team support at other sites as required.

To provide specialist clinical advice and phycological support to patients with complex wounds, liaising with and referring on to other healthcare professionals as required. The post holder will provide Tissue Viability and Pressure Ulcer prevention education and training to nursing and clinical staff across the site, including supporting the wider multidisciplinary teams with audit and evaluation. The post holder will support the development and implementation of the Trust Tissue Viability Strategy and support the development of systems, guidelines and policies that are evidence based. The post holder will deputise for the Lead Nurse for Tissue Viability, as required.

Clinical Practice

Be able to manage time effectively and identify priorities of care delivery and Tissue Viability Caseload.

Able to manage and maintain standards of the Tissue Viability Service; led by defined guidelines, protocols, procedures, codes of conduct and followed at own discretion and caseload responsibility.

Responding and escalating to changes in the needs and/or environment. Supporting in the planning to ensure effective use of staff and resources.

Refer to and work with multidisciplinary teams to develop and support the service user, family and carers; providing specialised advice on complex wounds and pressure prevention to facilitate the delivery of individualised care.

Uses numeracy, literacy, digital photography and technological skills to support safe and effective delivery of care.

Carries out required Tissue Viability nursing skills with dexterity and accuracy.

Carry out systematic and evidence-based assessments to give holistic care, taking into consideration physical, social, cultural, psychological, spiritual, genetic and environmental factors that may influence service user’s pathway or treatment.

Uses interprofessional and interpersonal skills to facilitate the best communication of highly complex, highly sensitive, and highly contentious information to patients, families/ carers, members of the public and workforce; where this may be challenging, unpredictable and potentially hostile.

Accepts accountability and responsibility for delivery of care and decisions within clinical practice, ensuring all care is delivered within scope of practice.

Maintains accurate, timely and legible records of documentation and care delivered, including how consent is obtained and whether the patient has capacity.

Will be able to provide and support staff to deliver safe wound care. Utilise clinical skills such as conservative sharp debridement, complex negative pressure wound therapy dressings and compression therapy to meet complex care needs dependent on competency, ensuring guidelines, policy and codes of conduct are adhered to.

Analyses, assesses, plans, implements and evaluates Tissue Viability care needs continuously, empowering the service user to achieve optimal health through developing and providing highly specialist packages of care and advice.

Advocate for patients’ best interests and respecting their moral values, beliefs, dignity and ethics.

Ability to analyse and interpret complex situations; using interpretation and assessment to mitigate risks and/or issues to patient/client clinical need; escalating to senior team when required.

Supports the need of the Trust to manage redeployment to support the provision of safe patient care and skill mix when identified by senior leadership teams. Ensuring to work within scope of practice and professional boundaries.

Holds a professional responsibility in Safeguarding vulnerable adults, young people and children, inclusive of patient’s valuables, adhering to Trust policies and protocols at all times.

Work with Medical equipment library to ensure correct use of pressure relieving equipment and that the equipment is in good working order and are within service calibration dates; escalating any damages and required maintenance using correct pathways. Influence the purchase or supply of pressure relieving equipment, making recommendation and decision on appropriate suppliers.

Ensure all data and information of the service user is treated with confidentiality, sensitivity and adheres with the Data Protection Act 2018, General Data Protection Regulation (GDPR) and the Caldicott Guidelines.

Acts as a professional role model demonstrating a leading example for values, behaviour, attitudes, conduct and appearance.

To maintain correct standards of uniform, identification and personal appearance, as indicated in the Uniform Policy and Dress Code.

Supports a diverse and inclusive environment and challenges any behaviours which are not in line with these behaviours to their managers.

Maintains a responsible and confidential use of social media platforms that do not bring the individual, profession, Trust, service users, colleagues or members of the public into disrepute.



Responsible for resources/equipment and promoting effective and efficient use; maintaining stock and supply control through procurement ordering.

Responsible for the day-to-day managing, planning and organisation of a team/service providing a broad range of activities and programmes within the speciality.

Works within the registrant’s professional code of conduct or alternative Governing Body and within organisational policies; contributing to changes in policy and practice changesarising from e.g. complaints or audits; impacting service and the wider organisation.



Facilitating Learning and Education

Welcomes and supports the learning and development of all staff and pre-registration learners on practice placement within clinical area, including members of the multidisciplinary team.

Facilitate a conducive learning environment to support best practice and safe patient care.

Engage with the mandatory annual appraisal process. Identifying individual learning needs, areas of strength/development and consider opportunities to support PDP.

Ensures all relevant E-Learning, mandatory, statutory and essential training is completed and compliance is maintained.

Engages in opportunities that expand knowledge, such as; utilising this knowledge to uphold and adopt best practice in the clinical area and share updates with the wider team.

Maintain professional revalidation, self-reflective practice as per registration requirements.

Maintain professional and accurate capability records and be able to produce original copies to demonstrate skills acquired.



Research and Innovation

Ensure all data and information of the service user is treated with confidentiality, sensitivity and adheres with the Data Protection Act 2018, General Data Protection Regulation (GDPR) and the Caldicott Guidelines.

To maintain professional duty of candour and be open, honest and transparent to your organisation, service users and their families/carers when incidents occur.

Use appropriate reporting systems and participate in encouraging a learning culture in response to investigations and outcomes.

Actively support the Trust, department and colleagues in sharing best practice and successes.

To maintain an awareness of the local and national targets, strategies and improvements being made in order to support care delivery.

Undertakes in service areas audits, R&D activities and clinical trials; applying outcomes and best practice to support and improve service provisions.

Proactive and contributes to the achievement of own clinical area/service quality improvements and to take ownership of role within this.
