Crynodeb
Work with individuals on a one-to-one basis to identify what matters to them and co-produce personalised support plans. Provide holistic support addressing social, emotional, and practical needs such as loneliness, housing, debt, employment, and lifestyle. Connect individuals to appropriate community groups, voluntary organisations, and local services. Build strong relationships with GP practices, community partners, and local stakeholders. Maintain accurate and timely records in line with PCN and NHS requirements. Contribute to service development, evaluation, and reporting.. Work in line with safeguarding, confidentiality, and information governance policies.
