Summary


Join us in an exciting role as a Senior IDT Nurse Assessor, where you’ll make a meaningful difference to patients with complex needs at a crucial point in their care journey.

Our Integrated Discharge Team works closely with patients and their families to ensure a safe, seamless transition from hospital to community. By anticipating needs and coordinating health and social care support, we aim to reduce unplanned admissions and promote high‑quality, patient‑centred care.

In this role, you’ll provide expert guidance, professional support and collaborative care planning—ensuring every patient receives an appropriate, well‑coordinated discharge plan without duplication of services. Your work will directly enhance patient experience and outcomes.

Based at Basildon Hospital, you’ll be part of a multidisciplinary network including social care and voluntary sector partners. Together, we champion integrated, holistic discharge pathways in line with the NHS Long Term Plan.

As a Senior Nurse Assessor, you will:

Support patients with complex healthcare needs, including those requiring long-term health and social care planning or short-term interventions following acute illness or changes in condition.

Assess patient needs and plan safe, effective, and timely discharges, working closely with the hospital multidisciplinary team (MDT) and external system partners to ensure robust discharge planning and maintain patient flow across the acute trust.

Provide effective leadership to junior members of staff, working alongside the Operational Lead/Team Manager to support, guide, and develop the team while fostering positive working relationships.

Play a key role within the Integrated Discharge Team, contributing to a collaborative model that brings together health and social care expertise to improve how care is delivered for patients.

Lead liaison and communication between the hospital and community services, including formal and informal carers, ensuring smooth, safe, and well-coordinated transfers between hospital and home (and vice versa).

Ensure high standards of patient-centred care, advocating for individuals’ needs and promoting safe, appropriate, and timely discharge pathways.
