Summary


We are seeking a bright, dedicated Nurse to join our Integrated Discharge Team Hub, supporting discharge planning across our four community hospitals.

This varied and rewarding role involves travelling between sites, so a valid driving licence and access to a vehicle for work purposes are essential.

Strong communication skills are essential, as you will work closely with patients, families, the multidisciplinary team, and wider system partners to coordinate care and deliver a high‑quality discharge experience.

This is a seven‑day service, including Bank Holidays, offering a dynamic and supportive environment where your contribution truly makes a difference.

You will:
• Work as part of the Integrated Discharge Team to support patients and families with complex needs, ensuring a smooth and well‑coordinated transition from hospital to the community.
• Provide professional guidance and support to patients and their families, working in partnership with them to develop the most appropriate and effective discharge plans.
• Ensure that all interventions are streamlined and not duplicated, contributing to improved care quality and increased patient satisfaction.
• Support patients with complex healthcare needs, including those requiring long‑term health and social care planning or short‑term intervention following an acute illness or change in condition.
• Anticipate, coordinate, and integrate health and social care needs for patients at high risk of unplanned hospital admissions.
• Take a central role in holistic care planning within Community Hospitals, collaborating closely with Social Care teams, voluntary sector partners, and other organisations.
• Assess patients who are medically optimised for discharge, identify their ongoing care needs, and make timely referrals to the appropriate community services.
• Play a key role in ensuring safe, effective, and timely discharges, supporting optimal patient flow throughout the hospital.
• Act as the primary liaison between the hospital and relevant community services—both formal and informal—to facilitate smooth and safe transfers between hospital and home.
