About the Role:
We are recruiting a dedicated Outreach Family Support Worker to provide high-quality, home-based Early Help support to children and families using a whole-family approach. This temporary long-term role involves working with children and young people aged 0–25* and their families, helping to prevent the need for statutory intervention and ensuring access to universal services.
You will take whole-case responsibility for complex families, coordinating support, completing Early Help Assessments, and acting as Lead Professional when required. The role includes providing intensive and targeted support, facilitating family engagement, and supporting children and young people with emotional and mental health challenges.
Key Responsibilities:
Case management for complex families stepping down from social care
Conduct Early Help Assessments, coordinate Team Around the Family (TAF) meetings, and monitor progress
Support children and young people with high-risk or complex needs
Deliver practical parenting strategies and de-escalation techniques
Maintain accurate records on electronic systems
Attend supervision, planning meetings, and professional reviews
Promote safeguarding, anti-discriminatory practice, and equality of opportunity
Support supervised contact sessions where required
Requirements:
Relevant Level 3 qualification in Health, Social Care, or Early Help (Level 5 preferred)
Experience working with children, young people, and families in a support or social care role
Knowledge of safeguarding, risk assessment, and family engagement methods
Solution-focused, family-centred approach (e.g., Signs of Safety)
Ability to work independently and as part of a multidisciplinary team
Excellent communication, record-keeping, and organisational skills
Enhanced DBS required
