Job Title: Practice Nurse Accountable to: Practice Manager (administrative) Nurse Manager (clinical) Hours: 24 hours per week (3 days, 8 hours per day) Job summary To deliver high-quality nursing care to the practice population, including treatment room services and long-term conditions management, using evidence-based practice. The post holder will work as part of the multidisciplinary team and contribute to infection prevention and control, clinical governance, and service development. The Practice will ensure access to appropriate clinical supervision and day-to-day support. Primary responsibilities Deliver treatment room nursing services including wound care, vaccinations, phlebotomy, ECGs, smears, and health checks Provide structured long-term conditions reviews in line with national guidance Assess patient needs and implement evidence-based care plans Maintain accurate clinical records and disease registers Support Infection Prevention and Control activity including audits, compliance monitoring, and annual IPC reporting Promote health education and disease prevention Support safeguarding and respond appropriately to clinical emergencies Liaise with external services to support vulnerable patients Secondary responsibilities Participate in audit and quality improvement activity Contribute to nursing protocols and clinical policy development Support shared learning within the team Participate in significant event reviews and governance processes Generic responsibilities All staff are expected to support: Equality, Diversity and Inclusion Health and Safety Confidentiality and information governance Quality improvement and service development Ongoing learning and professional development Effective multidisciplinary working Accurate clinical record keeping Professional conduct and security procedures
