Senior Clinical Fellow with interest in Urogynaecology Job overview Applicants are invited for a posts as Senior Clinical Fellows (ST6+ equivalent) in Obstetrics and Gynaecology with interest in Urogynaecology in Leeds Teaching Hospitals NHS Trust. This is an exciting opportunity to work in a friendly, dynamic, large, teaching acute hospital trust. Up to three posts are available. The post holder will work at a level equivalent to that of a ST6+ trainee. Applicants must have completed Foundation competencies either as part of the UK Foundation programme or demonstrate equivalent training. Applicants should ideally have experience at working at a ST6+ level for a minimum of 6 months ideally within the UK. Basic Life Support training is essential. Candidates will require current and valid GMC registration. Candidates will be encouraged to register and complete pelvic pain SITM. Main Duties Of The Job The above post is suitable for doctors wishing to gain clinical experience at a ST6+ level in Obstetrics and Gynaecology. The post offers the successful candidate a wide range of experience in all areas of the specialty. Whilst this post is not recognised for training, the appointee will be supported by a Clinical Supervisor to meet their career and educational goals. There will be opportunities to develop special interests in the following areas: Endometriosis: Clinical Fellow in Endometriosis (ST6+ Equivalent) BSGE-Accredited Endometriosis Centre Leeds Teaching Hospitals NHS Trust This post is for a Clinical Fellow in Endometriosis, suitable for doctors working at ST67 equivalent level, depending on prior experience and demonstrated competencies. The successful applicant will work within a British Society for Gynaecological Endoscopy (BSGE)accredited Endometriosis Centre, gaining advanced experience in the multidisciplinary management of complex endometriosis, including surgical, medical, research, and service-development aspects of care. The post carries a 60% time allocation with the Endometriosis Team, in addition to participation in: Full on-call commitments at LGI or St Jamess University Hospital (SJUH) One Antenatal Clinic (ANC) session per week This post is ideal for candidates wishing to develop expertise in endometriosis and progress towards subspecialist practice, advanced fellowship training, or consultant-level roles within endometriosis services. ________________________________________ Clinical Commitments and Responsibilities Endometriosis Service (60% allocation) The fellow will be fully embedded within the Endometriosis MDT and will participate in: Specialist Endometriosis Clinics o New and follow-up consultations o Complex counselling regarding medical and surgical management o Shared decision-making for advanced disease o Wil time the postholder will be encouraged to perform independent clinics Endometriosis Theatres o Exposure to advanced laparoscopic endometriosis surgery o Progressive operative responsibility according to experience and competence o Involvement in peri-operative planning and post-operative follow-up Multidisciplinary Management o Participation in Endometriosis MDT meetings: complex endometriosis, pelvic pain and functional urology. o Collaboration with colorectal surgery, urology, pain specialists, radiology, fertility services, and specialist nurses Thoracic Endometriosis Pathway o Opportunities for involvement in the regional thoracic endometriosis service, including MDT discussion, diagnostic pathways, and coordinated care ________________________________________ Research, Audit, and Service Development The post offers strong academic and quality-improvement exposure, including: Research opportunities o Involvement in ongoing endometriosis research projects o Support for audit, service evaluation, and presentation at regional or national meetings o Opportunity to contribute to publications where appropriate Audit and Quality Improvement o Endometriosis-focused audit and QI work aligned with BSGE standards o Opportunities to lead or co-lead service development initiatives The post holder will also be encouraged to take part in departmental teaching and quality improvement work. Working For Our Organisation The gynaecology service is predominantly based at St James University Hospital (Chancellor Wing) and includes general, urogynaecological, recurrent miscarriage, endometrial ablation, fertility control, complex contraception, menopause (based at a nearby GP practice), perineal and ambulatory gynaecology clinics such as hysteroscopy (including outpatient resection), vulval and colposcopy. Gynaecology theatres and the gynaecology acute treatment unit which includes an ultrasound scan service and outpatient acute gynaecological procedures, are also located here. Some elective gynaecology work including clinics, day case and overnight theatre and hysteroscopy also takes place at Wharfdale hospital. The fertility service is based at Seacroft Hospital. The maternity service is based at St James University Hospital (approximately 4,500 deliveries per year) and Leeds General Infirmary (approximately 5,200 deliveries per year). Each site has an antenatal day care unit, tertiary level fetal medicine unit, fetal assessment unit, obstetric ultrasound department and maternity assessment centre. Antenatal clinics include general high risk clinics as well as specialist ones including cardiac, diabetes, substance misuse, endocrine, epilepsy, teenage pregnancy, prematurity, obstetric haematology, placenta accrete spectrum and rheumatology clinic. Key Duties and Responsibilities The post holder will work on the second on call (ST6+) rota, and as such will take on the responsibilities associated with this role. The post holder will be supervised by a Consultant during clinical duties whether elective or on call and will be expected to work as part of a multidisciplinary team, demonstrating good communication skills with all members of the team. The weekly timetable will vary from week to week. It will include sessions in antenatal clinic, delivery suite, gynaecology outpatients, gynaecology theatre and the emergency gynaecology assessment unit. These sessions are aimed to diversify the post holders clinical experience, to provide a service and to accommodate the rolling on call rota of long days and night shifts on labour ward and the gynaecology acute treatment unit. On call frequency is 1 in 9. Antenatal clinic The post holder will be expected to: - Conduct the initial antenatal assessment of women, identify risk factors in the booking history, and consult or refer appropriately under the supervision of a senior registrar or consultant. - Counsel women adequately about the investigations offered during pregnancy, interpret the results, communicate the significance of abnormal results to the patient and arrange further management if needed. - Detect and manage commonly occurring disorders in pregnancy, recognising their limitations, and referring to other colleagues in the team when necessary - Recognise the roles of the multidisciplinary team - Demonstrate good communication with patients and staff Labour Ward The post holder will work under the supervision of a Consultant Obstetrician on call or on hotweek and: - Work in conjunction with the senior Midwife - Be aware of the wishes and concerns of women in labour, and maintain their dignity, autonomy, physical and psychological comfort - Help in monitoring of fetal condition in labour - Assess and manage abnormal progress in labour - Have a chance to perform instrumental and operative deliveries. - Assist in the management of third stage complications - Assist in the management of maternal collapse and neonatal resuscitation Gynaecology Outpatients The post holder will be allocated to attend specific Gynaecology clinics, but are also encouraged to attend a variety of the more specialised clinics which provide valuable learning opportunities when possible. The post holder will: - be able to take a gynaecological history and perform an appropriate examination with respect for the womans dignity and autonomy - initiate investigation and management of common gynaecological problems such as disorders of menstruation, infertility, incontinence, menopausal symptoms, psychosexual problems and genital tract infections. - understand cervical screening, the management of pre invasive and invasive disease and treatment follow up - Demonstrate good communication with patients and staff Gynaecology Acute Treatment Unit The post holder will work under the supervision of the Consultant Gynaecologist on call or on hotweek and: - Take referrals from general practitioners, emergency department and other specialities concerning women with early pregnancy or acute gynaecological conditions - Assess and initiate management and / or treat common early pregnancy and acute gynaecological conditions - Participate in acute gynaecology procedures including surgical management of miscarriage, manual vacuum aspiration in office setting, Word catheter insertion for Bartholins and laparoscopic surgery. - Review women after elective gynaecology surgery. Gynaecology Theatre The post-holder will observe, assist or undertake procedures in theatre with an appropriate level of supervision. On-call- The post-holder will always be resident in the hospital when on duty. The Consultant Obstetrician will be resident between 08.30-22.00 on weekdays and 08.30 and 17.00 on weekends. The Consultant Gynaecologist will be resident between 08.30 and 17.00 on weekdays and will conduct a resident ward round on weekends. They will remain resident beyond these times if required but will otherwise be on call from home. The post complies with the New Deal requirements and the European Working Time Directive.
