Crynodeb
To be responsible for ensuring that all CHC processes implemented are compliant with legislative and national guidance and in accordance with the responsibilities of the ICBs. To work in partnership with the Local Authority, Acute Trusts and Care Homes to ensure timely discharges and good quality and cost-effective placements. To implement a rolling programme of training for all continuing healthcare staff as well as partner organisations to achieve ongoing improvements in the system and keep abreast and disseminate policy changes. To prepare cases and reports in relation to any challenges to the established decision-making process, including supporting ICBs in instructing solicitors as required, up to and including judicial reviews. To work with the ICB Management, Head of Adult Social Care, Older People, Acute Hospital Discharge Management, Mental Health Teams, Out of Boroughs ICBs and Acute Hospitals to support the achievement of the Local Area Agreement target in relation to delayed transfers of care. Monitor staff performance and implement corrective action when required i.e. disciplinary procedures, sickness absence management, and grievance procedures.
