We are recruiting on behalf of a client for a full-time temporary role supporting individuals with a wide range of social, emotional and wellbeing needs. The postholder will work closely with people to understand their circumstances, develop tailored support plans and help them access appropriate services to improve their overall quality of life.
This is a people-focused role requiring empathy, strong listening skills and the ability to manage complex situations while empowering individuals to make positive changes.
Key Responsibilities
Deliver personalised wellbeing and social support to individuals referred through primary care or via self-referral pathways.
Carry out holistic, person-centred assessments to identify needs, priorities and goals.
Co-produce individual wellbeing plans that encourage engagement, independence and improved outcomes.
Act as a consistent point of contact, providing coordinated support throughout each individual’s journey.
Meet individuals on a one-to-one basis, creating a safe, supportive environment where people feel heard and respected.
Take a strengths-based, non-judgemental approach that values diversity, personal choice and lived experience.
Support individuals to explore and address wider factors affecting wellbeing, such as housing concerns, financial pressures, unemployment, social isolation or caring responsibilities.
Promote preventative wellbeing approaches and provide clear, accessible information to support informed decision-making.
Develop and maintain strong knowledge of local support services and community resources, proactively identifying new options that meet individual needs.
Work collaboratively with partner organisations to ensure effective, joined-up support.
Ideal Candidate Profile
Experience working in a wellbeing, social support, community or health-related role.
Strong interpersonal and communication skills with the ability to build trust quickly.
Confidence working with individuals facing complex or multiple challenges.
Organised, proactive and comfortable managing a varied caseload.
A compassionate, person-centred approach with a focus on empowerment and outcomes.
In return you must be able to start on 6th February be available for the duration of the role until 31st March 26
