Summary


Opportunities have arisen for a Community Assessment Officer within Salford's Adult Safeguarding Team. We are looking to recruit workers to help us deliver our community led support vision.

You will be responsible for supporting your manager in ensuring that people’s care and support needs are met through delivering high quality support. You will be the first point of contact for the person requesting support. You will undertake strength-based conversations with people, supporting them to maximise their wellbeing. You will offer a person centered approach to ensure that a compassionate and personalised approach is offered to the person you are supporting. You will work closely with other organisations to ensure that people are well connected, this will include supporting people to access statutory or voluntary services within their local community.

The Adult Social Care workforce aims to improve outcomes for adults who live in Salford by empowering individuals, adopting a strength’s-based approach across the service, promoting independence, and working in partnership with our residents so their experiences, opportunities and lives are as good as they can be. This will be achieved through developing a skilled and motivated workforce by investing in their continued professional development and ensuring their practice is current and of a high standard.

To maintain a high standard of work when carrying out statutory functions of behalf of Salford Adult Social Care. This includes assessing people under the Care Act (2014), focusing on connecting people with their local communities and promoting well-being. With appropriate support you will undertake assessments of a persons mental capacity giving regard to the specific decision which needs to be made. You will be required to complete support planning with individuals and their carers/families, and undertake reviews to ensure that the individual’s care and support needs are met and their well-being is promoted. You will be required to use the social care system to record appropriate documentation, including assessments, support plans, reviews and any other work which you are required to undertake. Home visits, social care assessments including assessing the caring responsibilities that the adult has for a child, to participate in MDT meetings, this could be with health, education or other professionals.
