Responsibilities
Conduct comprehensive assessments to identify clients' needs and appropriate interventions.
Develop, implement, and review care plans in collaboration with clients and relevant stakeholders.
Provide support and guidance to clients and their families, empowering them to make informed decisions.
Manage a caseload of complex cases, ensuring timely and effective service delivery.
Collaborate with multidisciplinary teams, including healthcare providers, educators, and community organizations, to coordinate services.
Maintain accurate and up-to-date case records in accordance with legal and organizational requirements.
Participate in regular supervision and professional development opportunities to enhance skills and knowledge.
Requirements
Valid Social Work qualification and full registration with the relevant professional body.
Minimum of three years post-qualification experience as a social worker.
Demonstrated expertise in working with individuals and families in a social care setting.
Thorough understanding of social work legislation, policies, and best practice standards.
Strong communication and interpersonal skills, with the ability to build rapport and trust with clients.
Proven case management skills, with the ability to handle complex and sensitive situations effectively.
Commitment to continuous professional development and staying current with emerging trends in social work.
Hours Per Week: 36.50
Start Time: 08:45
End Time: 17:15
Pay Per Hour: £17.99
